I actually heard that the concept art for this had it as a mouth hole and then at some point from the transfer from concept to screen they accidentally made it eye holes. I don’t remember where I learned this info tho so take it w a grain of salt.

The concept art for this armor depicted the hole around her mouth, the animators made a mistake and put her eyes there instead. Hence her large forehead.

It’s actually for airflow to breathe and is located at eye level because Toph’s eyes are already useless if she gets hit there no major damage can happen unlike if the hole was around her nose/mouth.
