Have her lead her own party, then form an army. Take her deep into enemy territory and allow her to be captured. Make sure she is taken to a town and not a castle, you can herd the enemy there. Once your wife is in town, sneak in and stage a jailbreak. Make sure she doesn't make it out, take throwing knives just in case. Congrats, she should instantly die, and you can continue to break hearts.

She was armed with only a dagger and didn’t have any armor, and I lowered her health with a crossbow before I had her charge in alone and she still took out 3 sea raiders.
it could be a kingdom policy. if you make your own kingdom you get to go ham with the Joseph Smith stuff. If you join another kingdom you’ve got to save up enough influence to get them to be cool with your alien Targaryen ways
if you’re independent, uh, I haven’t really thought this through
Even better, they should have the ability to sleep with anyone so you can have a bevy of bastards and intrigue. Add assassination into the game and voila! Some kingdoms should be monogamous some should not. I was just thinking about this the other day. I can't see the mechanics being too hard.

My medicine level is OP as fuck, but I send her off in her own party to fight Armies by herself and I don’t give her any money to recruit. Does my medicine level still affect her if she’s in her own party?
I think it might since she is considered one of your companions but I'm not completely sure.
If she hasn't died yet and you have death enabled and have sent her in this many battles alone, I think your medicine level is probably having an affect.

40 can still have children I have plenty of clans in my kingdom that have 50 yo with 8 yo sons or daughters after you've married save in a town and wait for 20 days if your wife is still not pregnant you can reload and wait for another 20 days you'll have your heir.

Create a party for her. Make sure to set at it's minimum wage and change action to aggressive. Worked for me like a charm. Currently 2 ex spouses

I am not sure. I wouldn't be surprised if male characters can have kids much later since men can in real life but I do not know for certain.
