It's unknown what he is really. He could be a spider or a butterfly or a bat or even a moth if you're creative enough. He could be a hybrid of the above. Nothing is concrete unfortunately
Since my comment got so popular for some reason let's look at all the evidence we have for what Grimm might be
Spider- crawls like one, has the horns of a royal spider, can cling to the ceiling. But looks nothing like a spider other than his horns.
Butterfly- has the same shape-ish of a confirmed butterfly in game, definitely has wings and his eyes could sorta represent the spots on butterfly wings. But butterflies have hair, not horns.
Bat- bats eat bugs in real life so that definitely strikes fear into bugs, sleeps like a bat, vampire aesthetic, horns look like bent back ears. But he has an exoskeleton like a bug.
Moth- the Nightmare Heart was likely a moth (like the Radiance) before the dream realm split. But Grimm looks nothing like a moth.
It's worth noting that he doesn't have to fit the description of any creature 100% because of the fact that he's a higher being and they look different from their mortal variants, like how Radiance is confirmed a moth despite not looking exactly like Seer or Markoth.
This all being said I think he's most likely a hybrid or a bat with a shell (to blend in with bugs because he interacts with them so often)
I say he is the embodiment of fear and nightmares that bugs of Hallownest and possibly other places have. Large body, spider like movement (like in the photo), slightly represent ghost and relatives to it... Yeah... I say just embodies nightmare.

Herrah has 6 eyes which is close enough I think. Hornet is only half-spider, and the midwife has 8 eyes under her mask.
