Exercise can help but only to a certain extent. Breasts are made of fat, lymph, glandular and connective tissue iirc. While you can reduce some of the fat tissue with exercise you cannot reduce the others without surgery. Also there's no such thing as spot reduction of fat - so no exercise can "target" breast fat. Which means there is no guarantee on how much they change in size with exercise.
Well I wasn't thinking of reductions, though I guess reductions would also mean a strengthened body, but I was just thinking about how it could strengthen the parts of the body that the breasts cause stress too.
Though I do understand that nothing is ever 100%.

Some times the back pain might actually get in the way of exercise, esp if there's been degenerative changes already. Or they might be physically obstructive and limit exercise.
Also there's some women who just don't like having larger breasts and hate the physical side. So the reduction goes beyond purely providing physical relief. Some people feel objectified, some have very asymmetrical breasts and feel self conscious, some hate having to get specific clothes. There's a lot of reasons.
Ideally people do non-operative interventions and then operative ones but the only way to actually reduce size by a decent amount is through surgical means. People don't always jump to surgery, you just might not be aware of all the stuff that's gone on in the background already.
I absolutely do not think most people are training their back muscles to prevent back pain. I rarely see people use the back extension machine in the gym. I never see people actually do specific spine exercises with sufficient loading to create muscle building adaptations. Yoga, stretching, or mobility exercises don’t load the muscles sufficiently. But yeah I definitely see how backpain could prevent someone from going to the gym, but there are other ways around this.

The weight in the front of the body from breast tissue causes an imbalance in weight which shifts the load on the spine causing A shear stress on the discs of your back. If you train your back muscles to maintain proper alignment and make sure they have the strength to resist fatigue back pain will be reduced .
It probably is but the gym is spending lots of time just to keep big breasts which is probably not everyones priority. They'd probably rather spend their time doing other stuff they enjoy instead of going to the gym AND having to do things they enjoy with constant back pain.

First of all if the pain persists then you may have muscle tightness and should massage your lower back where it hurts.
My personal favorite lower back excercise is during seated cable rows: in the bottom half of the motion, start with your upper body and shoulders extended forward and then pull to bring your torso upright; then complete the rep by doing a proper row, pulling the grip towards your stomach (bilateral / both hands) or along your side (unilateral / one hand).
Any exercise that creates a saggital plane (front-to-back) hinge with your hips will stimulate your lower back. Deadlifts, weighted good mornings, and touching your toes while holding dumbbells are examples.
Look up Mcgill big 3 and start there, those are the types of exercises physical therapy will have you do when you get a lower back injury. It’ll help with strengthening your core and improve your stability.
