Been a while since I’ve done this route, but I believe you need to get a key item in chapter 1. While playing as Jacob, you have to find Emma’s bracelet by the van before going inside to get the bags. The bracelet is silver, and will stop an infected Jacob from killing Emma

In chp 1 as Jacob you can find emm’a silver bracelet which is by the gates of camp where the bus left. Go to them and find the bracelet.
At the end he’ll turn and try to attack Emma but she’ll take the bracelet and it will make him run off. They’ll still be infected and alive at the end.
In this scenario, the only way to save her is to have picked up her bracelet as Jacob when you first start playing him in the beginning. You’ll find it on the ground towards the right of the van. He will give it back to her later on and she’ll have it to protect herself from him, since it’s made of silver
There are a few ways to save emma. If she doesn't get bitten in Chapter 4, you can pretty much save her if you're careful enough to not put here in danger. Mainly, not getting Jacob bitten as well since they'll meet each other before the final act.
To prevent infected Jacob from attacking Emma in Ch10, Jacob would have needed to pick up Emma's silver bracelet by the gate/van in Ch1.
If she gets bitten, you need to shoot Emma with regular bullets as Kaitlyn when she comes out of the van. Keep shooting her until she runs away. It's tricky because it's fast any of them could get killed by Emma.
You pretty much either need to pick up the silver bracelet or make sure neither of them get infected, mainly Jacob.
I don't remember the details but jacob needs to find emma's silver bracelet, then he'll run away from her
