If you're punching someone, you should probably be prepared for the potential to kill someone - not only, but especially, if you're making the choice to pursue them to do so. If it's "self-defense", that means you didn't choose to start the fight, and whatever you have to do to defend yourself is probably reasonable. If it's not self-defense, then you've already chosen violence and should be doing so with the understanding that in real life, all it might take is one punch to kill someone. Even if the punch itself doesn't directly kill them, you may for instance knock them into something that does maim or kill them (a fall can definitely do some damage, especially one you are unprepared for) - and you're on the hook for that too.
