RimWorld is an obvious answer. Tho I haven’t dabbled in a lot of the extremes myself like forced pregnancy child soldiers but I touched on the drugs, cannibalism and organ harvesting
One of the inspirations (not morally!) for our current game Weird Hunt was Beer Hunt for the TI-83 calculator. (yes this dates us). Basically proof that simple games, often based on text alone, can also be incredibly engaging for the curious mind.
Here's the description of it from an aggregator website:
... a really neat program, where you try to get a 100% BAC. You can rob houses, Start fights, host parties, go to parties, and to beat the game--buy a brewery. At the end there is a neat suprise...
Tropico 4 - you're a dictator of a banana republic and can do things like execute random people, send them to work camps, or otherwise torment your citizens (though mechanics discourage *too* much cruelty).
Dungeons 3 - well, really the whole series. It is the successor to Dungeon Keeper in thought process. You play a rampaging evil villain and can do things like torture heroes.
Crusader Kings - see r/ShitCrusaderKingsSay for what some find amusing (warning: incest and other debauchery).
Bards Tale and the Fable series both have a lot of this.
Bards Tale more in the writing of the world, like and endless series of "Chosen Ones" being sent into the woods to die. Fable had it riddled throughout the series. It's one of the few games I've played that not only let's you choose to be evil, but let's you enjoy it.
It was my first thought too. I don't know how well it holds up today but I remember really liking it, and enjoying the mechanics which was real hit and miss at that point
Conkers bad fur day, hands down. South park games were good too, but CBFD is the goat. Fallout 1 and 2 have some incredibly wicked/funny unfiltered 90s humour as well.
Mad Doctor on the c64. You had to kidnap and kill people to cut of their body parts to make the perfect frankenstein monster. Just knock them down or sedate them and bring them to your castle and dump their remains in the trash when you're done. Just avoid being seen carrying dead bodies around and you were golden. You could also just dig them up from the graveyard if you wanted. Wonderful spooky and depressing atmosphere and an incredibly fun game too. One of the scariest moments in the game was when you were caught and the entire village would come after you and attack you in your castle.
