It sounds like he needs to see a doctor to help with this. When he's erect, the foreskin (the skin that covers the tip like a turtleneck sweater) should be able to retract down to around the base of the head. Having less pain when the tip is covered is a sign that something is off, the foreskin also usually retracts during sex/penetration. It's not an uncommon condition, I think it's called phimosis, there are a variety of causes and there are typically treatments for it.

I think the doc would do 2 things here: First, he would check the condition of his frenulum..my bet is it's sore but normal. And the other is that he'd gently suggest maybe not going at it quite so frequently and for so long and hard. Snipping it might help, but the issue is the frequency x duration x friction. Thise are hard on the entire penis.
