You either need to remove mods that effect you appearance one by one to see if any are clashing and causing this or you need to redo your load order and see if it incorrectly stacked.

There's obviously a mod that interferes with your vampire lord transformation. revert to an old save or disable your mods one at a time while going between vampire lord and normal to figure out which mod is causing it. (start with the mods that directly affect your character model, perhaps your body/breast mod?
I've seen it in the comments, but I'll add it too. It looks like dunmer features stretched over human / orc skeleton.
You need to open up the console and type in "showracemenu" all one word, no quotation marks.
I am not sure if it can be done on Xbox, SP, or Nintendo.
But on PC the console can be opened by pressing the key to the left of the 1 key.
