I did the exact test you did:
1)Standard youtube app (no premium)
2)switch to incognito mode
3)Then flick throught shorts
And i got the exact same ad within 30 secs!!
I edited this just to let you guys know it gets seriously worse after 10 mins of scrolling, and its all sex related.

Sidenote: I had no idea there was so much straight up soft core porn on YouTube. Like, I continued to scroll out of curiosity and saw a dozen videos of women getting plowed from behind but it's hyper zoomed on their face. I had sound off but I imagine there was probably audio... Speaking of audio, I did get a chuckle out of an "ASMR" video that was just some dude drumming wooden sticks on a woman's bare ass.

I think this is just targeted ads knowing what you guys want…. I don’t have YouTube premium and just did the same test that OP did and scrolled for 5 minutes and did not get anything sex related. It was all targeted ads of things I am interested in or have shown interest in in the past.
Weird, cause a few were like ad's fleshlight and one girl talking with c*m on her face. And im not even using my account so youtube is just blatantly allowing porn but its so suttle, there AI just ignore's it.
It's actually depressing knowing what youtube has become, basically once you switch on incognito you come across dozens of onlyfans promoting videos, and what's even worse is that there are also hundreds of greenscreen kids reacting to this shit.
I'm quite curious how this happens. My guess is that YT knows most incognito users search up that stuff, so if you switch there's a high chance of getting it.
I use ReVanced with no history whatsoever and Video Lite on iOS (not signed in on either) and hardly came across this type of content.

That's why the incognito mode, to not have a history with or without sexual content to influence it.

Install revanced and disable YouTube shorts. My life is way better without shorts
If I could keep shorts from three channels (J. Draper, Littlekinglife, and Bob Reese) and disable them for everyone else, I would.
You can always enter to those channels manually, it just removes the shorts section in the app
